‘Heart Connect’ is a free information service exclusive to heart failure patients, offered over the phone.
The service compliments the medical care and support you provide to your patients.
Heart Connect offers support around
- coping with the psychological impacts of the condition
- reinforces education around symptom-management
- encourages the use of social supports
- signposting services.
